A case with gallbladder perforation due to behcet's disease and management by percutaneous drainge

Abstract

Background: Behcet’s Disease (BD) is a chronic, recurrent, systemic disease characterized by orogenital ulcers and oculocutaneous inflamatory lesions. The most frequent sites of gastrointestinal
involvement of BD are the ileocecal region and colon. Gallbladder perforation without any apparent cause is thought to be very rare, presents a diffucult problem.Case Report: Fourty-year-old man presented with 20 days history of generalised abdominal pain, nausea, vomiting and abdominal distension. In the past 10 days, he also noticed high fever. He had BD which was first diagnosed 13 years ago. Abdominal ultrasound examination showed fluid collection in subhepatic area which extended to left hypocondriac region. CT scan showed two fluid collections, one was in between liver and fundal part of gallbladder. The other was between middle pole of left kidney and lower pole of right kidney. Bothcollections were thought to be abcess. In view of thickened gallbladder wall, multiple fluid collections between liver and gallbladder and the abscess formations, gallbladder perforation was considered. US guided percutaneous subhepatic drain was inserted. Follow up US showed minimal pericholecystic fluid collection and sludge in gallbladder. The patient healed with this treatment. We have described an unusual case of gallbladder perforation asa complication of BD. We have not found any other risk factor for gallbladder perforation apart from vasculitis due to BD.Conclusions: BD should be kept in mind as a rare cause of gallbladder perforation. For the treatment of gallbladder perforation, surgery is a wellestablished prosedure. Percutaneous drainage is a choice of treatment of gallbladder perforation in selected cases.
