An unsual case of persistent inguinal swelling – beware immunodeficiency

Abstract
Background: Common variable immunodeficiency (CVID) is the most prevalent primary immunodeficiency in adults. Patients frequently present with recurrent sino-pulmonary infection. Here we discuss an unusual presentation of CVID to our surgical department.
Case Report: This report discusses the unusual presentation of inguinal pain and swelling leading to the diagnosis of this condition. A 61-year-old male presented to the Surgical department on several occasions with swellings in his groins, one of which was an abscess requiring drainage. Histological examination of a lymph node that was surgically excised on initial presentation was consistent with reactive hyperplasia. An ultrasound scan of his groins was carried out when he presented some time after this, and revealed what appeared to be reactive lymph nodes. Blood tests then confirmed the diagnosis of common variable immunodeficiency.
Conclusions: CVID is rare, but immunodeficiency disorders should be considered in all cases of recurrent lymphadenopathy when the diagnosis is not immediately obvious.
