Preoperative transesophageal echocardiogram (TEE) showing severe mitral regurgitation with color Doppler and systolic flow reversal of the pulmonary vein in a 43-year-old man with a history of systemic lupus erythematosus (SLE) who presented with endocarditis, which was later diagnosed to be an infection with Coxiella burnetii. The TEE was performed in 2018.
