Diagnostic and therapeutic problems in primary hyperparathyroidism - case report

Abstract

The 57-year-old woman has been treated for ischaemic heart disease, arterial hypertension, advanced osteoporosis and depressive states for a few years. Primary hyperparathyroidism was diagnosed on the basis of the results of calcium and phosphate serum concentrations and their 24-hour urine elimination as well as elevated parathormone concentration. Available localization techniques did not visualize parathyroid glands. It was decided to perform an explorative operation of the neck. Operator found four hyperplastic parathyroid glands. Three of them were removed completely and the fourth one - only in half. Temporary hypocalcaemia was observed during the postoperative phase. Summarizing: the explorative operation of neck conducted by a surgeon experienced in endocrinology should be carried out in the cases of unsuccessful localization of the hyperplastic parathyroid glands or parathyroid adenoma.
